New Patient Visit The initial visit is $300.00. This includes the doctor's time, and the non-invasive studies listed below. Typically this visit will focus on a single clinical complaint or concern. Complex clinical problems may take several visits to resolve. This initial visit is 75 minutes long, including the non-invasive tests.
Prior to the visit all patients must complete a secure on-line medical questionnaire. This will help to streamline your appointment and economize your time with the doctor. Please bring your current medications, including hormones, to the clinic for review at the time of your visit. Be sure to include a list of your supplements on the medical questionnaire.
All patients will receive two non-invasive studies at the beginning of the visit to assess your baseline vascular and neurologic health. Heart Rate Variability (HRV), Duplex Pulse Wave (DPA) Analysis, and Body Impedance Analyses (BIA) are FDA approved studies, and are invaluable in the comprehensive assessment of your state of health and physiology. A probe is placed on your finger, and EKG leads are applies to the hand and foot. Data is obtained as you sit quietly. Please arrive 15 minutes before your scheduled visit to obtain these studies to be incorporated into your visit with the doctor. Follow Up Visits Follow up visits depend on the type and amount of recommendations made by the doctor. The doctor’s billable rate per hour is $300.00 and is prorated to 30 minute blocks. All payment is required in full at the time of service. We are a cash-based practice, and accept cash, credit cards and checks. We accept Visa, Mastercard, Discover, Diners Club International, JCB and American Express. We do not accept insurance, and will not bill your insurance company. If you would like to submit a claim to your insurance company for reimbursement, of for application towards your deductible, we will provide you with the appropriate and customary codes that reflect the services performed. Medicare Please note that our office has "Opted Out" of the Medicare program. Medicare patients may not submit claims to Medicare for services rendered in the office. Testing CFM offers a wide variety of testing. While we make an effort to keep costs low, we also understand how expensive testing can be. We require that all patients have testing done with our affiliate labs (Genova Diagnostics, ZRT Labs, Dunwoody, US Biotek, Drs. Data, Diagnostics Solutions, SpectraCell, and Quest Diagnostics) so we can provide patients with optimal care. We reserve the right to make exceptions to this policy, and will do so based on consideration of individual needs. We strive to have all testing completed from labs that have relationships with our office. Patient Responsibilities You have scheduled a block of time with the doctor. It is your responsibility to arrive on time for your appointment. In respect for the following patients, all appointments will terminate at the scheduled time. You are expected to pay for the entire scheduled time, regardless of your arrival time.
All new patients must have a primary care provider to cover routine care, prescription refills, and emergencies. Chattanooga Functional Medicine will not be responsible for refilling existing prescriptions, but will refill prescriptions that were initiated at our practice. Prescription Refills Chattanooga Functional Medicine will only refill prescriptions that have originated in this office. Please allow up to one week for the office to refill these medications. The majority of our prescriptions are customized and are made by compounding pharmacies so please plan ahead and leave time for this process. Patients will be notified when the office is closed for holidays, inclement weather, and vacation time. Supplements, Nutraceuticals, Herbs We guarantee the providence and quality of all supplements sold in our practice, but cannot guarantee that the use of herbs, nutrients and supplements will “cure” your medical conditions. All therapeutic recommendations are made in your best interest; your health and safety are our foremost concerns. All supplement sales are final. Telephone Calls and Email Communications The office staff has been trained to address most questions, however if you have a medical question this needs to be addressed with the doctor. We can schedule brief phone consultations which are billed out as a fraction of the hourly rate.
We try to answer all communications in a timely fashion. If you do not get an answer, we are directly involved in patient care. Please leave a voice mail and your call will returned by the end of the day, or the following morning. We have a receptionist that works in the office on Mondays, Tuesdays, and Thursdays. Please note that while the office is open on Wednesdays and Fridays, reception hours are limited. Emergencies CFM is not a primary care clinic, and does not provide primary care services. It is a specialty clinic providing subspecialized care plans and cutting edge medical thinking. All patients are required to have a primary care physician. In the event of a medical emergency do not call our clinic. Please call your primary care physician or dial 911. Tune In!
